How It Works
Febuxostat is a non-purine, selective xanthine oxidase (XO) inhibitor.

XO is an enzyme involved in converting hypoxanthine → xanthine → uric acid (final step of purine metabolism).

By inhibiting XO, febuxostat reduces uric acid production, thereby lowering serum uric acid levels.
Dosage
As prescribed by the physician based on the severity of hyperuricemia and response to treatment. Usual doses are 40 mg or 80 mg once daily.

Drug Interactions
❗ Use caution or avoid use with:

Naproxen and other glucuronidation inhibitors

Indications

Chronic treatment of hyperuricemia in gout patients.
Contraindications / When Not to Use
❌ Do not use in:

Known hypersensitivity to febuxostat or excipients
-
Patients taking azathioprine, mercaptopurine, theophylline
-
Asymptomatic hyperuricemia
Precautions
-
Avoid in patients with ischemic heart disease or congestive heart failure.
-
Monitor renal function in those at risk.

Use with caution in elderly or those with hepatic impairment.
Warnings
⚠️ Warning 1
-
Gout flares may increase after starting treatment. This is due to mobilization of urate crystals.

Prophylactic NSAIDs or colchicine are advised when initiating febuxostat.
⚠️ Warning 2
-
Use in patients with malignancy, Lesch-Nyhan syndrome, or those at high risk of xanthine accumulation is not recommended.
⚠️ Warning 3

Liver function tests (LFTs) should be done at 2 and 4 months post-initiation and monitored periodically thereafter.
