Description

Glimepiride is an oral blood glucose-lowering drug of the sulfonylurea class. Its primary mechanism of action is stimulating insulin release from pancreatic β-cells. It also increases insulin sensitivity in peripheral tissues and enhances insulin binding to its intracellular receptor. Long-term glucose-lowering action may involve additional yet unclear mechanisms.


Uses / Indications

🔹 Used as an adjunct to diet and exercise for improving glycaemic control in adults with Type 2 Diabetes Mellitus
🔹 Not effective and not indicated for Type 1 diabetes or diabetic ketoacidosis


Dosage

Note: Always follow your doctor’s dosage instructions.

  • Starting dose (Adults):
    Typically 1–2 mg once daily, with breakfast or the first main meal

  • Maintenance dose:
    May be titrated up to a maximum of 8 mg/day, based on glycemic response

  • Renal impairment:
    Use with caution and initiate at the lowest possible dose


How to Use

  • Take the tablet orally once daily, with food

  • Swallow whole with water

  • Do not crush or chew

  • Monitor blood sugar levels regularly


Overdose

Symptoms of overdose may include:

  • Severe hypoglycemia (sweating, shakiness, confusion, fainting)

  • Seek immediate medical attention


Missed Dose

  • Take the missed dose as soon as you remember

  • If it's near your next dose, skip the missed dose

  • Do not double the dose to compensate


When Not to Use

❌ In patients with:

  • Type 1 diabetes mellitus

  • Diabetic ketoacidosis (with or without coma)

  • Known hypersensitivity to glimepiride or other sulfonylureas

  • History of allergic reaction to sulfonamide derivatives


Side Effects

🟡 Common Side Effects:

  • Hypoglycemia

  • Dizziness, headache, weakness, nausea

🔵 Gastrointestinal:

  • Vomiting, abdominal pain, diarrhea (rare)

🔴 Rare but Serious:

  • Elevated liver enzymes

  • Allergic skin reactions (rash, itching, urticaria)

  • Hemolytic anemia in G6PD-deficient patients

  • Severe allergic reactions (anaphylaxis, Stevens-Johnson syndrome)


Drug Interactions

💊 May enhance Glimepiride's effects (↑ hypoglycemia risk):

  • Salicylates, sulfonamides, chloramphenicol, monoamine oxidase inhibitors, probenecid, coumarins, beta-blockers

💊 May reduce effectiveness (↑ blood sugar):

  • Thiazide diuretics, corticosteroids, thyroid products, oral contraceptives, phenothiazines, phenytoin, estrogens, nicotinic acid

⚠️ Beta-blockers may mask hypoglycemia symptoms


Precautions & Warnings

🔶 Warning 1:
Hemolytic anemia can occur in patients with G6PD deficiency
→ Use non-sulfonylurea alternatives in such patients

🔶 Warning 2:
Increased cardiovascular mortality risk associated with sulfonylureas
→ Inform patient of risks and consider alternatives in high-risk individuals

🔶 Warning 3:
Hypersensitivity reactions (e.g., anaphylaxis, angioedema, Stevens-Johnson Syndrome)
→ Discontinue immediately if reactions occur


Precautions

✔️ Monitor blood glucose regularly
✔️ Educate patient on hypoglycemia signs and how to manage
✔️ Caution in elderly and patients with renal/hepatic impairment
✔️ Use with other antidiabetics should be closely monitored


Food Interactions

  • Take with meals to reduce risk of hypoglycemia

  • Avoid alcohol, which may increase risk of low blood sugar


Pregnancy Category

  • Always consult your physician before use during pregnancy

  • Insulin is generally preferred in pregnant diabetic patients
