Usage and Safety

When not to Use (Contraindications)
Do not use in patients with:

Severe left ventricular dysfunction

Hypotension (SBP < 90 mmHg) or cardiogenic shock

Sick sinus syndrome (unless patient has a functioning pacemaker)
Precautions

Verapamil is extensively metabolized in the liver → use cautiously in patients with impaired hepatic function.
Warnings

May lower blood pressure excessively → risk of dizziness or symptomatic hypotension.

Has negative inotropic effect (weakens heart contraction), but usually balanced by reduced afterload.

Liver enzyme elevations (transaminases, alkaline phosphatase, bilirubin) have been reported in some patients.
