Usage and Safety
Dosage:

Side Effects

Common: Constipation, dizziness, nausea, headache, fatigue, edema, flushing.

Cardiac: Bradycardia, AV block, angina pectoris, atrioventricular dissociation, palpitations, syncope, CHF (congestive heart failure), myocardial infarction (rare).

When not to Use (Contraindications)
Do not use in patients with:

Severe left ventricular dysfunction

Hypotension (SBP < 90 mmHg) or cardiogenic shock

Sick sinus syndrome (unless patient has a functioning pacemaker)
Precautions
-
Verapamil is extensively metabolized in the liver → use cautiously in patients with impaired hepatic function.
Warnings
-
May cause a drop in blood pressure below normal → dizziness or symptomatic hypotension may occur.

Has a negative inotropic effect (weakening of heart contraction), but usually balanced by reduced vascular resistance.
-
Elevated liver enzymes (transaminases, alkaline phosphatase, bilirubin) have been reported in some patients.
