How it works
Nebivolol is a ß1-selective adrenoceptor antagonist with vasodilating properties.

Its vasodilating action is attributed to interaction with the L-arginine/nitric oxide pathway, which is unique compared to other beta-blockers.

It lacks intrinsic sympathomimetic and membrane-stabilizing activity at therapeutic doses.

At clinically relevant doses, nebivolol does not block α1-adrenergic receptors.

Dosage:

Side Effects

Common: Headache, dizziness, paresthesia, dyspnea, constipation, nausea, diarrhea, tiredness, edema.

Uncommon: Nightmares, depression, impaired vision, bradycardia, heart failure, AV block, hypotension, intermittent claudication, bronchospasm, dyspepsia, flatulence, vomiting, pruritus, rash, erythema, impotence.

Rare: Syncope, aggravated psoriasis.

Contraindications (When not to Use)
Do not use in patients with:

Hypersensitivity to Nebivolol or any excipients

Severe hepatic insufficiency
-
Acute heart failure, cardiogenic shock, or decompensated heart failure requiring IV therapy

Sick sinus syndrome (including sino-atrial block)
-
Second or third degree heart block (without pacemaker)
-
History of bronchospasm or bronchial asthma
-
Untreated pheochromocytoma
-
Metabolic acidosis
-
Bradycardia (HR < 60 bpm)
-
Hypotension (systolic BP < 90 mmHg)
-
Severe peripheral circulatory disturbances
Precautions
-
Use cautiously in patients with compensated congestive heart failure.

If heart failure worsens, consider discontinuation.
Warnings

Diabetes & Hyperthyroidism: May mask symptoms of hypoglycemia or hyperthyroidism (e.g., tachycardia).

Abrupt Withdrawal: Avoid sudden discontinuation—taper dose gradually (halve weekly). Sudden stop may worsen angina or heart failure.

Peripheral Vascular Disease: May aggravate symptoms of arterial insufficiency; use with caution.
