How it works
Nebivolol is a ß1 selective adrenoceptor antagonist whose hemodynamic effects differ from those of classical ß-adrenoceptor antagonists as a result of its vasodilating action. This mild vasodilating effect is attributed to its interaction with the L-arginine/nitric oxide pathway, a property not shared by other ß-blockers. Nebivolol lacks intrinsic sympathomimetic and membrane-stabilizing activity at therapeutically relevant concentrations. At clinically relevant doses, it does not demonstrate a1-adrenergic receptor blockade activity.

Side Effects

  • Common: Headache, dizziness, paresthesia, dyspnea, constipation, nausea, diarrhea, tiredness, edema.

  • Uncommon: Nightmares, depression, impaired vision, bradycardia, heart failure, slowed AV conduction/AV-block, hypotension, intermittent claudication (increase), bronchospasm, dyspepsia, flatulence, vomiting, pruritus, rash, erythematous reactions, impotence.

  • Rare: Syncope, worsening of psoriasis.

Drug Interactions

  • Calcium channel blockers (verapamil, diltiazem)

  • Antiarrhythmic agents (disopyramide)

  • Digitalis glycosides and other ß-blockers

  • Reserpine, guanethidine, clonidine

  • CYP2D6 inhibitors (quinidine, propafenone, fluoxetine, paroxetine)

  • Cimetidine

  • Sildenafil


Indication

  • Essential hypertension

  • Stable, mild to moderate chronic heart failure (in addition to standard therapies in elderly patients ≥70 years)

  • May be used alone or in combination with other antihypertensive agents


When not to Use

Contraindicated in patients with:

  • Hypersensitivity to nebivolol or any component

  • Severe hepatic insufficiency

  • Acute heart failure, cardiogenic shock, or episodes of decompensated heart failure requiring IV inotropic therapy

  • Sick sinus syndrome, including sino-atrial block

  • Second/third degree heart block (without pacemaker)

  • History of bronchospasm or bronchial asthma

  • Untreated phaeochromocytoma

  • Metabolic acidosis

  • Bradycardia (HR < 60 bpm before treatment)

  • Hypotension (SBP < 90 mmHg)

  • Severe peripheral circulatory disturbances


Precautions

  • In compensated congestive heart failure, nebivolol should be used cautiously.

  • If heart failure worsens, discontinuation should be considered.


Warnings

Warning 1: In diabetic patients, nebivolol may mask hypoglycemia symptoms (tachycardia, palpitations). ß-blockers may also mask tachycardia in hyperthyroidism. Abrupt withdrawal may intensify symptoms.

Warning 2: Treatment should not be stopped abruptly; taper gradually (halving weekly). If angina worsens or acute coronary insufficiency develops, reinstitute nebivolol at least temporarily.

Warning 3: ß-blockers may worsen peripheral arterial insufficiency; use with caution in patients with peripheral vascular diseases.
