How it Works

Nebivolol is a ß1-selective adrenoceptor antagonist (beta-blocker) with unique properties:

  • Provides vasodilation via the L-arginine/nitric oxide pathway, unlike traditional beta-blockers.

  • Reduces heart rate and cardiac workload.

  • Lacks intrinsic sympathomimetic or membrane-stabilizing activity at therapeutic concentrations.

  • Does not block alpha1-adrenergic receptors at clinical doses.


Usage and Safety

Dosage: As directed by physician (Nebivolol HCl, typically once daily).


Side Effects

Common:

  • Headache, dizziness, paresthesia

  • Dyspnea, constipation, nausea, diarrhea

  • Tiredness, edema

Uncommon:

  • Nightmares, depression, impaired vision

  • Bradycardia, heart failure, AV-block, hypotension

  • Bronchospasm, intermittent claudication

  • Dyspepsia, flatulence, vomiting, rash, pruritus

  • Impotence

Rare:

  • Syncope, worsening psoriasis


Drug Interactions

  • Calcium channel blockers (verapamil, diltiazem)

  • Antiarrhythmic agents (disopyramide)

  • Digitalis glycosides

  • Other ß-blockers, reserpine, guanethidine

  • Clonidine

  • CYP2D6 inhibitors (quinidine, propafenone, fluoxetine, paroxetine)

  • Cimetidine

  • Sildenafil


Indications

  • Treatment of essential hypertension

  • Treatment of stable, mild to moderate chronic heart failure (elderly ≥70 years, with standard therapies)

  • Can be used alone or with other antihypertensive agents


When Not to Use

Contraindicated in patients with:

  • Hypersensitivity to Nebivolol or excipients

  • Severe hepatic insufficiency

  • Acute heart failure, cardiogenic shock, or heart failure decompensation needing IV inotropes

  • Sick sinus syndrome, SA block

  • 2nd or 3rd degree heart block (without pacemaker)

  • History of bronchospasm / asthma

  • Untreated phaeochromocytoma

  • Metabolic acidosis

  • Bradycardia (<60 bpm before therapy)

  • Hypotension (SBP <90 mmHg)

  • Severe peripheral circulatory disorders


Precautions

  • Use cautiously in compensated congestive heart failure; discontinue if condition worsens.


Warnings

Warning 1:

  • May mask hypoglycemia symptoms in diabetics.

  • May mask tachycardia in hyperthyroidism.

  • Avoid abrupt withdrawal to prevent rebound symptoms.

Warning 2:

  • Discontinue gradually (halve dose weekly).

  • If angina worsens or acute coronary insufficiency develops, reinstitute therapy promptly.

Warning 3:

  • May aggravate arterial insufficiency in peripheral vascular disease patients.
