How it works
Piroxicam, an NSAID complexed with cyclodextrin, possesses analgesic and antipyretic properties. The complex allows a single molecule of NSAID to be released adjacent to the gastrointestinal mucosa instead of as crystals. Since the contact time with the gastric mucosa is reduced, the risk of direct gastric irritation is also minimized.


Usage And Safety

Dosage
Piroxicam beta cyclodextrin — as prescribed by a physician.

Side Effects

  • Very common: Nausea, epigastric distress, abdominal pain, discomfort, flatulence, constipation, diarrhea.

  • Other possible reactions: Hypersensitivity signs (skin rash, headache, vertigo, asthenia), blood chemistry changes, increased blood urea.

  • Less common: Vomiting, allergic edema (face, hands), blurred vision, tinnitus, aplastic anemia, leukopenia, eosinophilia, pancytopenia, thrombocytopenia, liver function parameter changes, jaundice, acute renal insufficiency, water retention (e.g., ankle edema), hypertension, congestive heart failure, gastric ulcer with perforation, Stevens-Johnson’s syndrome, Lyell’s syndrome, agranulocytosis, bladder disorders, shock, acute heart failure, stomatitis, alopecia, nail growth disorders.

  • Rare: Gastric ulcers and hemorrhages.

Drug Interactions
Warfarin, protein-bound drugs, methotrexate, aspirin & other NSAIDs, plasma lithium concentrations, diuretics, anti-hypertensives, cardiac glycosides, quinolone antibiotics, mifepristone, cyclosporine, aminoglycosides, oral hypoglycemic agents.

Indication
Used for acute painful conditions requiring anti-inflammatory and analgesic activity, including rheumatoid arthritis, osteoarthritis (arthrosis, degenerative joint disease), ankylosing spondylitis, musculoskeletal and joint disorders, gout, soft-tissue disorders, and post-operative pain.

When not to Use
Contraindicated in:

  • Hypersensitivity to piroxicam.

  • Gastroduodenal ulcer, gastritis, dyspepsia.

  • Severe hepatic or renal disturbances, severe heart failure, severe hypertension, severe blood alterations, hemorrhagic diathesis.

  • NSAID/aspirin-induced asthma, rhinitis, or urticaria.

  • Pregnancy, lactation, and children.

Precautions
Use with caution in patients with asthma due to possible bronchial smooth muscle spasm aggravated by prostaglandin inhibition.


Warnings

  1. NSAIDs inhibit renal prostaglandin synthesis, which supports renal perfusion in patients with reduced renal blood flow and volume.

  2. Serious gastrointestinal toxicity (bleeding, ulceration, perforation) can occur anytime, with or without warning symptoms. Use under strict medical control in patients with a history of upper GI tract issues.

  3. Piroxicam may antagonize the antihypertensive effect of thiazide diuretics and β-blockers — monitor patients with hypertension closely.
