How it Works:
Amodiaquine is a Mannich base 4-aminoquinoline, structurally and mechanistically similar to chloroquine. It is metabolized to desethylamodiaquine, which accumulates inside the parasite’s food vacuole and disrupts haem detoxification. It also inhibits the glutathione-dependent destruction of ferriprotoporphyrin IX in the malaria parasite, causing toxic peptide buildup. Amodiaquine is active against some chloroquine-resistant strains, though some cross-resistance exists.

Dosage:
As prescribed by a physician (based on body weight and malaria type).

Side Effects:

  • Abdominal discomfort

  • Nausea, vomiting

  • Headache, dizziness, blurred vision

  • Weakness, fatigue

  • Itching

  • Cardiovascular abnormalities

  • Dyskinesia, ocular damage

  • Neuromuscular disorders, hearing loss

Drug Interactions:

  • ↑ Plasma concentration & liver enzymes with efavirenz

  • ↓ Plasma concentration with nevirapine

  • ↑ Risk of neutropenia with zidovudine or trimethoprim + sulfamethoxazole

  • ↑ Risk of cardiac events with anti-arrhythmic agents

  • Visual disturbances with mefloquine

  • Use caution with CYP2C8 & CYP2A6 inhibitors

  • May inhibit CYP2D6 & CYP2C9

Indication:
Used (with artesunate) for uncomplicated P. falciparum or P. vivax malaria. Also effective against P. ovale, P. knowlesi, and P. malariae.

When Not to Use:

  • Hypersensitivity to 4-aminoquinolines

  • Hepatotoxicity or hepatic disease

  • Alcoholism

  • Patients on hepatotoxic drugs

  • Neutropenia

  • Retinopathy

Precautions:

  • G6PD deficiency

  • Nerve disorders

  • Allergies

  • Kidney/liver impairment

  • Pregnancy & breastfeeding — only under medical supervision

Warnings:

  1. Avoid long-term use — risk of vision loss.

  2. Use cautiously in hepatic disease or when taking hepatotoxic drugs.

  3. Monitor serum ALT if asthenia & vomiting occur.
