How it works

  • Valsartan: Blocks angiotensin II from binding to its receptors, preventing vasoconstriction and aldosterone release, leading to reduced blood pressure.

  • Amlodipine: A calcium channel blocker that inhibits the movement of calcium into cardiac and vascular smooth muscle cells, causing relaxation and vasodilation.

  • Hydrochlorothiazide: A thiazide diuretic that inhibits sodium and chloride reabsorption in the distal renal tubules, increasing urine output and reducing blood volume, thereby lowering blood pressure.

Usage And Safety

Side Effects

  • Blood disorders: agranulocytosis, bone marrow failure, hemolytic anemia, leukopenia, thrombocytopenia, aplastic anemia.

  • Metabolic changes: hypercalcemia, hyperglycemia, hyperlipidemia, hyperuricemia, hypokalemia, hypomagnesemia, hypochloremic alkalosis.

  • Neurological: headache, hypertonia, visual disturbances.

  • Cardiac: arrhythmias (including bradycardia, ventricular tachycardia, atrial fibrillation).

Drug Interactions

  • Amlodipine: CYP3A4 inhibitors (ritonavir, ketoconazole, itraconazole, erythromycin, clarithromycin, verapamil, diltiazem), CYP3A4 inducers (carbamazepine, phenobarbital, phenytoin, rifampicin, St. John’s Wort), sildenafil, simvastatin, immunosuppressants, dantrolene, grapefruit.

  • Valsartan: Potassium-sparing diuretics, heparin, NSAIDs, rifampicin, ciclosporin, ritonavir, lithium.

  • Hydrochlorothiazide: Metformin, antidepressants, antipsychotics, antiepileptics, bile acid sequestrants, cyclosporine, lithium, alcohol, barbiturates, narcotics, amantadine, beta blockers, cytotoxic agents, digitalis glycosides, iodine contrast agents, methyldopa, tubocurarine.

Indication
Treatment of essential hypertension in adults.

When not to Use

  • Hypersensitivity to any active ingredient or related derivatives.

  • Second and third trimester of pregnancy.

  • Severe hepatic impairment, biliary cirrhosis, or cholestasis.

Precautions
Use with caution in patients with unilateral/bilateral renal artery stenosis or stenosis to a solitary kidney. Monitor blood urea and creatinine levels.

Warnings

  1. Pregnancy Risk: Drugs acting on the renin-angiotensin system can cause fetal harm or death in 2nd/3rd trimester.

  2. Hypotension Risk: May occur in volume/salt-depleted patients; correct condition before starting therapy.

  3. Renal Risk: May cause acute renal failure in some patients.
