How it Works

Terbinafine is an allylamine with broad-spectrum antifungal activity. At low concentrations, it is fungicidal against dermatophytes, moulds, and certain dimorphic fungi, and may be fungicidal or fungistatic against yeasts, depending on the species.
It selectively inhibits the enzyme squalene epoxidase, blocking fungal sterol biosynthesis at an early stage. This causes ergosterol deficiency and squalene accumulation within the fungal cell membrane, leading to fungal cell death.

Side Effects

Very common (≥1 in 10):

  • Loss of appetite, stomach ache, indigestion, bloating, nausea

  • Diarrhoea

  • Itching, rash, swelling

  • Muscle and joint pain

Common (≤1 in 10):

  • Headache

Uncommon (≤1 in 100):

  • Taste loss or disturbance (usually reversible within weeks after stopping; rare cases may be prolonged and lead to weight loss, anxiety, or depression)


Drug Interactions

  • Rifampicin

  • Cimetidine

  • Antidepressants (TCAs, SSRIs, MAOIs)

  • Oral contraceptives (possible irregular bleeding)

  • Beta blockers, antiarrhythmics (propafenone, amiodarone)

  • Warfarin

  • Ciclosporin

  • Other antifungals (fluconazole, ketoconazole)

  • Dextromethorphan

  • Caffeine


Indications

Treatment of fungal infections of:

  • Groin area

  • Skin (ringworm)

  • Toenails and fingernails (onychomycosis)

  • Feet (athlete’s foot)


When Not to Use

  • Allergy to terbinafine or any tablet component

  • History of liver problems

  • Breastfeeding


Precautions

Use with caution in patients with lupus erythematosus (rare cases reported).


Warnings

  1. Liver disease – Contraindicated in chronic or active hepatic disease. Liver function tests required before starting therapy.

  2. Serious skin reactions – Rare but severe (Stevens–Johnson syndrome, toxic epidermal necrolysis). Discontinue if rash progresses.

  3. Blood disorders – Rare cases of neutropenia, agranulocytosis, thrombocytopenia, pancytopenia. Evaluate cause and consider discontinuation.
