Generic Name: Lisinopril

Used For: Hypertension


How it Works

Lisinopril is a peptidyl dipeptidase inhibitor that inhibits the angiotensin-converting enzyme (ACE), responsible for converting angiotensin I to angiotensin II — a potent vasoconstrictor. By blocking this conversion, it suppresses the renin–angiotensin–aldosterone system, leading to vasodilation and reduced blood pressure.
It is effective even in low-renin hypertension.


Usage and Safety

Dosage: Lisinopril (as prescribed by a physician)


Side Effects

Common:

  • Dizziness

  • Headache

  • Orthostatic hypotension

  • Cough

  • Diarrhea, nausea, vomiting

Uncommon:

  • Mood changes, paraesthesia, vertigo, taste disturbance, sleep disorders

  • Myocardial infarction or stroke (possibly secondary to hypotension in high-risk patients)

  • Palpitations, tachycardia, Raynaud’s phenomenon, rhinitis

  • Abdominal pain, indigestion, rash, pruritus, impotence, fatigue, asthenia

  • Increased blood urea, serum creatinine, liver enzymes, hyperkalaemia

Rare:

  • Decreased hemoglobin/hematocrit

  • Mental confusion, dry mouth

  • Angioedema (face, extremities, lips, tongue, glottis, larynx)

  • Urticaria, alopecia, psoriasis, uraemia, acute renal failure

Very Rare:

  • Bone marrow depression, anemia, thrombocytopenia, leucopenia, agranulocytosis, haemolytic anemia

  • Lymphadenopathy, autoimmune disease, hypoglycemia

  • Bronchospasm, sinusitis, allergic alveolitis, eosinophilic pneumonia

  • Pancreatitis, intestinal angioedema, hepatitis (hepatocellular or cholestatic), jaundice, hepatic failure

  • Severe skin reactions (pemphigus, TEN, Stevens–Johnson syndrome, erythema multiforme)

  • Oliguria/anuria, gynecomastia, hyponatremia, increased serum bilirubin


Drug Interactions

  • Diuretics

  • Potassium supplements or potassium-sparing diuretics (e.g., spironolactone, triamterene, amiloride)

  • Potassium-containing salt substitutes

  • Lithium

  • NSAIDs and other ACE inhibitors

  • Glyceryl trinitrate & other nitrates

  • Tricyclic antidepressants & antipsychotics

  • Sympathomimetics

  • Antidiabetic medicines (insulin, oral hypoglycemics)


Indications

  • Hypertension: Treatment of high blood pressure

  • Heart Failure: Symptomatic management

  • Acute Myocardial Infarction: Short-term (6 weeks) treatment in haemodynamically stable patients within 24 hours of onset

  • Diabetic Nephropathy: Renal protection in hypertensive type 2 diabetic patients with early nephropathy


When Not to Use

Contraindicated in patients:

  • Hypersensitive to lisinopril, any excipients, or any ACE inhibitor

  • With history of ACE inhibitor–associated angioedema

  • With hereditary or idiopathic angioedema

  • Who are pregnant


Precautions

Discontinue immediately if angioedema occurs; treat with antihistamines/corticosteroids and monitor until full resolution.


Warnings

  1. Risk of Hypotension: Most likely after first doses or dose increases; higher risk with diuretics, salt restriction, dialysis, dehydration, diarrhea, or vomiting. Monitor closely.

  2. Cardiac Outflow Obstruction: Use with caution in patients with left ventricular outflow obstruction.

  3. Surgery/Anesthesia: May block angiotensin II formation; if hypotension occurs, correct with volume expansion.
