How It Works

Lornoxicam works by inhibiting the synthesis of prostaglandins through the inhibition of the cyclo-oxygenase (COX) enzyme. This reduces inflammation and pain without increasing leukotriene production. Like other NSAIDs, the full analgesic mechanism is not completely understood.

Indications

Lornoxicam is indicated for:

  • Short-term treatment of moderate pain (e.g., after dental surgery)

  • Mild to moderate pain from extra-articular inflammation

  • Pain associated with acute lumbo-sciatica

  • Symptomatic relief of pain and inflammation in osteoarthritis and rheumatoid arthritis


Side Effects

Common:

  • Abdominal pain, diarrhea, dyspepsia

  • Nausea, vomiting

  • Headache, dizziness

  • Increased blood urea nitrogen, creatinine, liver enzymes (ALT, AST), alkaline phosphatase

Uncommon:

  • Constipation, dry mouth, gastritis, flatulence, dysphagia

  • Peptic ulcers, gastrointestinal bleeding

  • Stomatitis, hemorrhoidal bleeding

  • Thrombocytopenia, increased bleeding time

  • Anemia, alopecia, rash, urticaria, dermatitis, pruritus

  • Increased sweating, purpura, flushing

  • Edema, hypertension, palpitations, hypotension, tachycardia

  • Drowsiness, vertigo, tremor, taste perversion

  • Dyspnea, bronchospasm, cough, rhinitis

  • Insomnia, somnolence, agitation, depression

  • Liver function abnormalities

  • Myalgia, leg cramps

  • Vision disorders, conjunctivitis, tinnitus

  • Appetite or weight changes

  • Aseptic meningitis, malaise, weakness


When Not to Use

Do not use Atcam (Lornoxicam) in the following situations:

  • Hypersensitivity to Lornoxicam or any component

  • Allergic reactions to NSAIDs (e.g., asthma, rhinitis, angioedema, urticaria)

  • Active or history of peptic ulceration or GI bleeding

  • Cerebrospinal bleeding or other bleeding disorders

  • Severe liver impairment, renal impairment (creatinine >700 µmol/L), or heart insufficiency

  • Severe thrombocytopenia

  • Elderly patients >65 years and weighing <50 kg undergoing acute surgery


Precautions & Warnings

Precautions

  • Gastrointestinal Monitoring: Regular monitoring is recommended in patients with history of GI ulcers or bleeding. Discontinue treatment if GI complications occur.


Warnings

Warning 1 – Renal Impairment:

  • Mild impairment (creatinine 150–300 µmol/L): Monitor quarterly

  • Moderate impairment (creatinine 300–700 µmol/L): Monitor every 1–2 months

  • Discontinue if renal function deteriorates

Warning 2 – Coagulation Disorders:

  • Close clinical monitoring and laboratory assessment (e.g., PTT) required

Warning 3 – Liver Diseases:

  • For patients with liver conditions (e.g., cirrhosis), regular liver enzyme monitoring is necessary


Drug Interactions

Avoid or monitor with:

  • Sulfonylureas

  • Other NSAIDs & Aspirin

  • Loop & Thiazide diuretics

  • ACE inhibitors

  • Lithium

  • Methotrexate

  • Cyclosporine

  • Cimetidine

  • Digoxin

  • Drugs affecting CYP2C9 enzymes such as:

    • Phenytoin

    • Amiodarone

    • Miconazole

    • Tranylcypromine

    • Rifampicin
