How It Works
-
Clopidogrel inhibits platelet activation and aggregation by irreversibly binding to the P2Y12 class of ADP receptors on platelets.
-
Aspirin irreversibly inhibits cyclooxygenase-1 (COX-1) by binding its acetyl group to a serine residue, preventing the conversion of arachidonic acid to thromboxane A2 (TXA2), a potent inducer of platelet aggregation.

Warning 1
-
Monitor for bleeding, including occult bleeding, especially in the first weeks of treatment or after cardiac procedures/surgery.
-
Avoid use with oral anticoagulants unless absolutely necessary.
Warning 2
-
Acquired haemophilia may develop with clopidogrel. If aPTT is prolonged (with or without bleeding), suspect acquired haemophilia.
-
Discontinue clopidogrel and refer to a specialist.
Warning 3
-
In patients with recent transient ischaemic attack or stroke, combination therapy (aspirin + clopidogrel) increases major bleeding risk.
-
Use with caution unless proven beneficial in a clinical setting.
