โœ… Uses / Indications

Adfolic OD is used in the following conditions:

  • Pregnancy & lactation to prevent neural tube defects

  • Folate deficiency (including that caused by epilepsy medications)

  • Prevention of spontaneous abortions

  • Hyperhomocysteinemia

  • Chronic kidney dialysis or long-term RBC damage

  • Cognitive disorders like dementia, depression, and Alzheimer’s disease

  • Dietary supplementation for adults and elderly

  • Patients with increased folate requirement or poor folic acid absorption


โš™๏ธ How It Works

Quatrefolic (5-MTHF) is the active form of folic acid, bypassing conversion steps required by folic acid. It:

  • Participates in nucleotide synthesis (DNA/RNA building)

  • Converts homocysteine to methionine with Vitamin B12

  • Supports amino acid metabolism

  • Reduces risk of hyperhomocysteinemia, a factor in cardiovascular and neurological diseases

๐Ÿšซ When Not to Use

Avoid Adfolic OD in:

  • Untreated Vitamin B12 deficiency (e.g. pernicious anemia)

  • Known hypersensitivity to folate or Quatrefolic

  • Long-term use in elderly without cobalamin absorption testing

  • Malignant disease, unless folate deficiency is a complication

  • Addisonian or macrocytic anemia unless B12 is supplemented

๐Ÿง  Nervous System:

  • Headache, dizziness, agitation, seizures

  • Confusion, hallucinations, psychosis

  • Drowsiness or coma (rare and dose-dependent)

๐Ÿงด Skin / Immune Reactions:

  • Rash, urticaria, erythema, pruritus

  • Stevens-Johnson syndrome, anaphylaxis, toxic epidermal necrolysis

โš ๏ธ Others:

  • GI symptoms: Nausea, flatulence, anorexia, abdominal distension

  • Peripheral edema, fever


๐Ÿ’Š Drug Interactions

May reduce effectiveness of:

  • Fosphenytoin, Phenytoin, Phenobarbital, Primidone (↑ seizure risk)

  • Pyrimethamine (↓ antiparasitic efficacy)

  • Methotrexate (↓ anticancer or immunosuppressive effect)

May be reduced by:

  • Sulfasalazine (↓ folic acid absorption)


โš ๏ธ Precautions & Warnings

โš ๏ธ Precaution:
  • B12 deficiency must be corrected before folate use

  • Uncorrected B12 deficiency may lead to neurological damage

  • Monitor vegetarians, infants, or patients with megaloblastic anemia

โš ๏ธ Warnings:
  1. Folate-dependent tumors may worsen with folic acid

  2. High doses may worsen seizures in epilepsy patients

  3. Not for general supplementation in healthy pregnant women (lower doses advised)
